“How long is the implant going to last?”. Most common question after price. Short answer : many years, often a lifetime. Long answer depends on factors you can actually control.

The real number : 90-95 % success at 10-20 years

The most rigorous long-term clinical studies (10 and 20 year follow-ups) show 90-95% success rates in well-selected patients. Meaning : the vast majority of implants placed today will still be in place in 2046.

In many cases, the implant (titanium root in bone) lasts the patient’s whole life. What may need replacement is the crown or prosthesis on top, generally at 10-15 years.

Three components, three durations

When we say “implant”, three things are mixed with different lifespans :

  1. The implant (titanium root) : usually lasts decades, often a lifetime.
  2. The abutment (intermediate piece) : rarely changed, only if broken.
  3. The crown (visible tooth on top) : 10-15 years on average. Replaced without touching the implant.

This matters : even if the crown needs replacement at some point, the implant itself remains valid. It is the most expensive part and the one that lasts longest.

What makes an implant last

1. Implant quality and planning

A serious implant brand (with 20-30 years of scientific literature) + careful planning (bone quantity and quality, precise position) are the foundation. No saving here.

2. Impeccable daily hygiene

Implants do not get cavities, but the gum around can become inflamed (mucositis) and the infection can progress to the bone (peri-implantitis). It is the “implant version” of periodontitis, and the first cause of medium-term failure.

Minimum daily routine :

  • Electric brushing twice a day.
  • Floss or interdental brush at least once a day (preferably at night).
  • Water flosser if your dentist recommends.

3. Professional check-ups every 6 months

Professional cleaning + implant-specific review + periodic X-rays. Early detection of any mucositis sign = fast treatment = implant saved.

4. Do not smoke

Smoking significantly reduces implant success rates. It reduces initial healing and long-term gum health. If you smoke and want implants, ideally quit (at least during the first weeks post-surgery).

5. Treat bruxism

If you grind or clench your teeth (bruxism) frequently, implants receive excessive forces. Night guard = mandatory. Without it, the implant can suffer and the crown can fracture before time.

6. Controlled diabetes and general health

Poorly controlled diabetes reduces healing and peri-implant health. Other conditions (osteoporosis with bisphosphonates, active chemotherapy, etc.) require specific evaluation.

What reduces duration

By order of impact :

  1. Poor hygiene : the first cause of medium-term failure.
  2. Smoking : significantly reduces survival.
  3. Untreated bruxism : can fracture the crown or overload the implant.
  4. Poorly controlled diabetes.
  5. Lack of professional check-ups for years.
  6. Deficient initial planning (rare with an experienced implantologist).

And if the implant fails : what happens

It is exceptional, but can occur. In most cases :

  • The implant is removed.
  • The area is treated and healing is awaited.
  • A new implant is placed (sometimes a bone graft is needed first).

The good news : a new placement, under better conditions, usually succeeds.

The long-term investment
