A dental emergency on holiday is stressful, even more so abroad. The good news : in tourist-heavy areas like the Costa Blanca, English-speaking dental care is widely available, often same-day. Here is the calm, practical guide.

First : how to tell if it is a real emergency

These are the situations that should not wait :

  • Severe toothache that does not respond to paracetamol or ibuprofen.
  • Significant facial swelling, fever, or difficulty swallowing.
  • Broken or knocked-out tooth after a fall or impact.
  • Lost crown, filling or bridge exposing a vulnerable tooth.
  • Persistent bleeding that does not stop with 15-20 minutes of pressure.
  • Trauma from sports, swimming pool accidents, etc.

Anything else (mild sensitivity, gum bleeding while flossing, etc.) can wait until a regular visit.

Immediate actions according to the problem

Severe toothache

  1. Take ibuprofeno 400 (available over the counter at any Spanish farmacia, look for the green cross sign). 1 every 6-8 hours.
  2. Apply cold to the cheek from outside, in 10-15 minute intervals.
  3. Rinse gently with warm salt water.
  4. DO NOT take antibiotics without prescription. They do not relieve acute pain.
  5. Call an English-speaking dentist for same-day appointment.

Broken or knocked-out tooth

  1. Pick up the tooth holding it by the crown, never by the root.
  2. Keep it in cold milk or saline (if available). If neither : in your saliva. Never in tap water.
  3. Go to the dentist immediately. For a fully knocked-out tooth, the first 1-2 hours are critical to re-implant.
  4. If the dentist is closed, go to the nearest hospital A&E (Urgencias).

Significant facial swelling / abscess

  1. Cold on the cheek, never heat (heat makes infection worse).
  2. Standard painkillers as above.
  3. If fever, difficulty swallowing or breathing : go to hospital A&E (Urgencias) directly.
  4. Otherwise : call your dentist for same-day appointment.

Lost crown or filling

  1. Keep the crown if it fell out whole.
  2. Avoid chewing on that side.
  3. Soft food for the next 24 h.
  4. Call the dentist within 24-48 h.

What pharmacies in Spain can give you

Spanish farmacias have a wide range of OTC products :

  • Ibuprofeno 400 mg (the standard for dental pain).
  • Paracetamol 1 g (if you cannot take ibuprofen).
  • Mouth gel with eugenol (clove oil) : temporary topical relief.
  • Hydrogen peroxide rinses for gum care.

Pharmacies are usually open Monday to Friday 9-14h and 17-21h, and at least one is always on call (“farmacia de guardia”). Look for the green cross sign with hours posted on the door.

What pharmacies cannot give you without prescription :

  • Antibiotics (require prescription).
  • Strong painkillers.

Look for clinics that explicitly mention :

  • “English-speaking team”
  • “Dental emergencies”
  • “Same-day appointments”

Avoid the generic “find a dentist” apps that may send you to clinics with limited English.

For severe medical emergency (breathing, severe trauma)

Call 112 (European emergency number, English-speaking operators).

For minor issues that can wait

A regular dental appointment within a few days is fine.

What if it is the weekend or out of hours

  • Saturday-Sunday : most Spanish clinics are closed. Some have on-call emergency services - check before. Hospital A&E (Urgencias) can manage acute infection and basic pain control but rarely definitive dental treatment.
  • Late evening / night : same as above. Pain control + monitoring until clinic opens Monday.

Insurance : what to keep

Whatever you do, keep all paperwork :

  • Receipt of the consultation.
  • Written diagnosis (the dentist can provide one).
  • Receipt of pharmacy products.
  • Photos of the issue if relevant.

Submit to your insurance on return. Most travel insurance reimburses emergency dental up to a cap (typically £200-£500).

Avoiding the next one

If you spend long periods in Spain :

  1. Have a “Spain dentist” identified before any problem arises. Save the contact in your phone.
  2. Annual check-up while you are there can prevent issues.
  3. Carry basic painkillers in your travel kit (paracetamol + ibuprofen).
  4. Avoid biting hard objects (ice, pen caps, etc.).
